Icotrokinra Sustains High-Impact Site Clearance at 2 Years

Updated

Icotrokinra sustained clearance of high-impact anatomical sites through two years of treatment, according to interim results from the ICONIC-TOTAL open-label extension study reported by HCPLive.

High-impact sites — the scalp, face, palms, soles and genital area — drive much of the disease burden in plaque psoriasis because visible and functionally critical skin resists standard topical therapy and responds less predictably to systemic agents. Clearance in these regions, sustained rather than achieved briefly, has become a key differentiator among newer systemic options.

The two-year ICONIC-TOTAL data extend the previously reported induction-period findings for icotrokinra, an investigational oral selective TYK2 inhibitor that blocks the IL-23 and IL-17 signaling axis upstream of the cytokines directly implicated in psoriasis pathogenesis. Johnson & Johnson is developing the molecule.

What does the 2-year extension show?

The open-label extension enrolled patients who completed the parent ICONIC induction studies, allowing investigators to observe whether response in high-impact sites persists once the induction phase ends. The reported outcome is durability: patients who achieved clearance of high-impact anatomical sites maintained that clearance through the 104-week mark.

Open-label extension designs carry known limitations. Without a concurrent placebo arm, durability and safety estimates reflect the treated population that chose to continue, and investigator scoring is unblinded. Even so, two-year maintenance of high-impact site clearance in a real-extension setting is a meaningful signal for a psoriasis therapy, because relapse in scalp, palmoplantar and genital skin is common with older oral agents.

Why high-impact sites matter

Regulators and clinicians increasingly treat high-impact site involvement as a distinct endpoint rather than a subset of overall body-surface-area improvement. Skin in these regions carries disproportionate effects on work, sexual function and quality of life, and patients with exclusively high-impact disease may fall below traditional severity thresholds while still warranting systemic therapy.

For an oral agent, durable high-impact site clearance positions icotrokinra against both biologic competitors and the older oral standard, apremilast, whose efficacy in these anatomical regions has historically been more limited.

What comes next

The ICONIC-TOTAL results add to the icotrokinra development program as Johnson & Johnson advances the molecule through late-stage evaluation in plaque psoriasis. Full presentation of the two-year dataset — including efficacy magnitudes and safety detail beyond the headline durability finding — is expected at an upcoming medical congress, where reviewers will scrutinize whether the sustained high-impact site clearance holds across the full extension cohort.
